In the past this has
usually meant that one or more of the boxes on the reg form was missing. Usually you have to put something in every box even if it is a "No thanks" or a dummy pdga member number if you don't have one (which I know is not the case for you).
Just a thought if Steve hasn't gotten back to you already.
